LVN or LPT PM Shift

PM Shifts Currently Available:
Generally, Wednesday-Saturday 1pm-11pm with Sunday, Monday and Tuesday off.

Qualifications

Licensed nursing staff shall hold a valid LPT, LVN, or RN license and perform the following nursing services within the scope of their license.

Job Duties

Services shall include but not be limited to the following:
  • Involvement in the planning and implementation of each client's care plan including assessment and documentation.
  • Obtaining and documenting physician orders for medical care, laboratory work-ups and administration of medication.
  • Monitoring of client's height, weight, and vital signs.
  • Writing weekly and monthly progress notes.
  • Notifying the physician promptly of any sudden marked adverse change in signs, symptoms, or behavior exhibited by a client.
  • Due to the unpredictable nature of the psychiatric profession employees are to engage in crisis prevention procedures for the safety, health, and welfare of the residents and staff.
  • In addition to specific job duties, due to the nature of the psychiatric field, at times, you will be expected to stand for long periods of time, bend, kneel, lift, carry, and walk swiftly or run.

Team Leader

Qualifications

Team leaders will have graduated from high school or obtained a G.E.D. (General Education Diploma) and have a minimum of two years full time work experience (or it’s part time equivalent) working in a mental health program serving persons with severe and persistent mental disabilities.

A Bachelor's degree in Psychology, Social Work, or Behavioral Science may be substituted for two years of experience.

Team leaders will report to the Program Director.

Job Duties

  • Under the direct supervision of the Psychiatrist and Program Director, employees will be responsible for the treatment, counseling and supervision of California Psychiatric Transitions’ residents.
  • Assure that proper documentation occurs whenever necessary including weekly, monthly, and quarterly progress summaries and incidental charting.
  • Assist in identifying behavior problems and develop and implement behavioral care plans.
  • Update care plans as required.
  • Meet regularly with the Program Director to discuss treatment problems and progress.
  • Communicate with the Mental Health caseworker and family when necessary.
  • Facilitate team meetings and groups assigned by the Program Director that allow residents to discuss their needs and progress.
  • Assure that the clothing needs of the residents assigned to them are met.
  • Complete monthly and quarterly written reviews of each resident assigned.
  • Communicate each resident’s progress to the facility psychiatrist.
  • Staff each resident that is assigned with facility psychiatrist on a monthly basis.
  • Track each resident’s group hours.
  • Track each resident’s program level and inform and review with their psychiatrist when a resident qualifies to change levels.
  • Act as a liaison between the resident and treatment. Route resident requests through the appropriate administrative channels.
  • Other duties as assigned by the Program Director.

Activities Assistant

Qualifications

All rehabilitation staff shall have graduated from high-school or possess a General Equivalency Diploma (GED) and have a minimum of two years full time work experience (or it’s part time equivalent) working in a mental health program serving patients with severe and persistent mental illness or developmental disabilities.

A Bachelor’s degree in Social or Behavioral Science can be substituted for two years of experience. Bachelor’s Degree preferred. Compensation will be commensurate with experience.

The Activities Assistant at California Psychiatric Transitions will be under the supervision of the Program Director and Activities Coordinator.

Job Duties

  • Develop and implement creative, expressive, educational, psychosocial, recreational, and activity-based groups that will effectively engage residents and support their treatment plans.
  • Monitor residents on outings to support successful integration and involvement with the community and drive a 15-passenger van if qualified to do so.
  • Assist with vocational rehabilitation program to help residents learn and practice work-related skills, and document vocational rehab progress monthly.
  • Work collaboratively with all departments to further develop programming for residents and assist on the floor when needed.
  • Plan and implement themed events, as well as holiday festivities.
  • Provide therapeutic counseling support in all aspects of position, and effectively use de-escalation skills as needed.
  • Document therapeutic progress made by residents on a quarterly basis, in addition to documenting incidents and psychiatric symptoms as they occur.
  • Attend staff meetings and in-service training as designated or required.
  • Due to the unpredictable nature of the psychiatric profession, employees are to engage in crisis prevention procedures for the safety, health, and welfare of the residents and staff.
  • Meet with residents individually on an as needed basis to facilitate activities.

Dietary Aide

Qualifications

1 year experience in dietary or therapeutic meals, i.e. caloric conscious meals (especially in a hospital or other similar facility), is required.

Preferred experience in kitchen preparatory work, cooking, & line assembly serving techniques.

Preferred Serve Safe Certified.

Job Duties

  • Employees will be trained in basic food sanitation techniques, food preparation, kitchen clean up, and maintaining the dietetic services areas.
  • Employees will also be trained to prepare each item on the posted menu including special diets (prescribed by an MD) and appropriated portions (as indicated by an MD).
  • Employees will be informed of appropriate clothing and attire to be worn in the kitchen, such as a cap or hair net. Employees will also be required to closely crop, trim, or cover beards and mustaches.
  • Due to the unpredictable nature of the psychiatric profession employees are to engage in crisis prevention procedures for the safety, health and welfare of the residents and staff.
  • In addition to specific job duties, due to the nature of the psychiatric field, at times, you will be expected to; stand for long periods of time, bend periodically, kneel, lift/carry up to more than 25 pounds, and walk swiftly or run.

Mental Health Worker

The Mental Health Worker position has the most direct client contact at our facility. MHW's make up the largest department and work alternating weekly schedules of three 12-hour and four 12-hour shifts.

Qualifications

All rehabilitation staff shall have graduated from high-school or possess a General Equivalency Diploma (GED).

Preferred One year of experience with direct patient or client care in a mental health, medical, or human services setting.
**OR**
60 college semester units. (Desirable - 15 semester units in behavioral sciences.)

Job Duties

  • Help develop, plan and implement rehabilitation groups/activities.
  • Assist in socialization and community integration groups/activities.
  • Insure client safety and comfort.
  • Provide resident supervision and advocation.
  • Counsel residents on the activities of daily living.
  • Conduct daily groups and encourage participation.
  • Chart signs and symptoms of resident behavior.
  • Report behavioral changes to medication and document all unusual occurrences.
  • Chart on 1:1 and Q15 monitoring.
  • Document residents daily progress in their plan of rehabilitation.
  • Attend in-service training as designated or required.
  • Note all worthy events pertaining to resident care or treatment.

Housekeeping Attendant

Qualifications

Preferred experience in housekeeping (especially in a hospital or similar facility) but not required.

Job Duties

  • The Housekeeping position requires qualified individuals to maintain high standards of cleanliness and safety for the facility. This includes resident bedrooms, bathrooms, dining areas, offices, and all common areas indoor/outdoor.
  • Housekeeping is also responsible for sorting, drying, and folding the laundry.
  • Interested candidates must be flexible with their schedules including weekends.
